Napoleon’s European Wars in 1792 saw the birth of the flying ambulance – or “flying ambulance”, which used a system of horse-drawn carriages to transport victims to hospitals for treatment and medical assistance. The system was also in effect during the American Civil War, but one of the earliest precursors to modern emergency medical services dates back to before World War I. Ambulance services were operated by local funeral homes, using hearses to transport patients to the hospital. Understandably, the attendants had very little first aid training at the time, unlike modern ambulance services which are both technologically advanced and staffed with highly trained paramedics.

At the time, ambulances were simply used as means of transportation to take patients to the nearest hospital. This soon proved insufficient, as many patients would eventually succumb to death along the way. Eventually, several medical services began devising ways for patients to receive preliminary medical care in an ambulance, bringing them instant relief and drastically reducing the number of deaths. According to the West Virginia Department of Education, New York’s emergency services in 1869 had a response time of 30 seconds and were equipped with an ambulance surgeon and a quart of brandy for patients, to help numb the pain.

Although the first gas-powered ambulance was the Palliser Ambulance introduced to Canada in 1905, the First World War saw the advent of motorized ambulances on a larger scale. After the war, ambulances began to carry two-way communication devices and radio dispatchers to reduce the time needed to provide assistance. At that time, advances in the treatment of bleeding wounds and broken bones also saved several lives and restored injured limbs.

It wasn’t until the 1970s that ambulances began to resemble those of the modern world. Roof height was increased, along with ambulance carrying capacities, and sirens were made mandatory, along with tail lifts, ramps, winches and stretchers. The 1980s, on the other hand, saw the emergence of formal legislation focusing on pediatric and geriatric care.

In India, the first example of establishment of EMS services was in Mumbai, in 1985, when 15 ambulances were connected to a centralized dispatch center wirelessly by the Association for Trauma Care of India.
